The Southern Tier End of Life Coalition envisions a community of people and organizations engaged in helping persons achieve an experience of dying that reflects each individual’s personal wishes for physical and emotional comfort, dignity, meaning and respect.

 

Family Health Care Decisions Act

 

The Southern Tier End-of-Life Coalition is pleased to announce that the Family Health Care Decisions Act is now the law of New York State.

 

This law gives legal standing to family members in making decisions for their loved ones who cannot participate in medical decision making and who did not appoint a Health Care Proxy prior to losing the ability to make their own decisions.

Contrary to popular perception, the Family Health Care Decisions Act makes it more important than ever to appoint a Health Care Proxy and to instruct that person on what your wishes are for the end of your life.

 

Failure to appoint a Health Care Proxy means that someone on the list of surrogate decision makers may follow their own values in making medical decisions rather than following your wishes.

 

Please join us in celebrating this new law and insure that your Health Care Proxy is readily available. In addition, it is essential that your Proxy is easily found in the event that you are not be able to make your own decisions about your medical care.

Should I fill out Health Care Proxy and Advance Directive Forms?
If you are age 18 or older, you should have advance directives in place. Whether you are in perfect health, or fighting terminal illness, advance directives are the only way to ensure your voice will be heard in the event of an accident or medical emergency. Your family and spouse do not have a legal say in your health care unless you assign someone as your health care proxy.

 

How/where can I obtain the forms for my Health Care Proxy and Advance Directives?
A variety of acceptable forms are available online.
