The Southern Tier End of Life Coalition envisions a community of people and organizations engaged in helping persons achieve an experience of dying that reflects each individual’s personal wishes for physical and emotional comfort, dignity, meaning and respect.

SOME QUESTIONS TO ASK YOURSELF...Should I fill out Health Care Proxy and Advance Directive
Forms?
If you are age 18 or older, you should have advance directives in
place. Whether you are in perfect health, or fighting terminal illness,
advance directives are the only way to ensure your voice will be heard
in the event of an accident or medical emergency. Your family and
spouse do not have a legal say in your health care unless you assign
someone as your health care proxy.
How/where can I obtain the forms for my Health Care Proxy
and Advance Directives?
