Should I fill out Health Care Proxy and Advance Directive Forms?
If you are age 18 or older, you should have advance directives in place. Whether you are in perfect health, or fighting terminal illness, advance directives are the only way to ensure your voice will be heard in the event of an accident or medical emergency. Your family and spouse do not have a legal say in your health care unless you assign someone as your health care proxy.

How/where can I obtain the forms for my Health Care Proxy and Advance Directives?
A variety of acceptable forms are available online.
